Question regarding SSS Salary Loan Monthly Amortization

SSS Form ML1: Monthly Salary/ Calamity/ Educational/ Emergency/ Stock Investment Loan Payment Return

The Monthly Loan Payment Return Form ( SSS Form ML-1) can be used by a self-employed, voluntary or employer-member of the SSS to report payment of his monthly salary, calamity, educational, emergency and stock investment loan amortizations or those of his employees.

You must fill up Form ML-1 using black ink and submit it in three copies to any SSS-accredited bank or to the Cashiering Department at the SSS head office at East Avenue, Diliman, Quezon City, together with your payment. This form must be submitted together with SSS Form ML-2 (Collection List) to ensure proper posting of payments.
Self-employed nga pala ako, and this is my first time to have Salary loan from SSS. Actually, nakuha ko na last May yun Check. Problem ko na lang ay monthly payment. Nalilito lang ako kung ano ilalagay ko sa form yun sa bandang right with penalty, etc. Or paki-fillup nalang kahit sample lang. hehe.

    thanks!

don't know if its the same if self employed or as employer paying for employees. me kasi yun gumagawa nito sa amin eh... i think it's pretty much the same.

they will mail the ML-2 to you then may naka-attach na ML-1 but not similar to the pic you posted. yun nga lang, late!

you can get copies of the ML-1 at SSS offices [like the one you posted] and use that to pay. yun kasama ng ML-2, u wont need it na.

you can pay even w/o the ML-2. you just need these when you will post your quarterly payments sa SSS office.

if wala pa yun latest ML-2, just photocopy the previous month's ML-2 then change the month. ok lang yan. just make sure to have 3 ML-2s with you when you go to the SSS office, kung hinde, hinde nila tatanggapin.

sorry, hinde me marunong pano mag edit nung pic eh... so type ko nalang. hehe.

    payment type = salary loan
applicable month = month year [june 2007]
enter total amt....

better siguro if per month mo nalang pay, para hinde magulo kasi, pag mali gawa mo, papaulit ulit pa yun mga forms sa yo. or if u want, quarterly. just do it at the beg of each quarter para walang penalty. kasi yun posting is quarterly eh para hinde magulo.

or, if may iba kang gusto na style, better to ask them first para hinde magka mali. toxic kasi magkamali dyan eh, mabusisi masyado.

    hth.
